Description:
Initially the title (165) was bound to achievement ID (3037) "Observed (25 player)" https://www.wowhead.com/achievement=3037/observed-25-player
In later patches (7.3.5) the 10-man and 25-man versions were removed and the achievement (12399) bacame only "Observed" https://www.wowhead.com/achievement=12399/observed

The new achievement (12399) is obtainable, however it does not grant the title (165) https://imgur.com/a/eVl2FaE
The achievement sub-text confirms it should grant the title (seen in the screenshots)

DELETE FROM `achievement_reward` WHERE `entry`='12399';
